- CHILDREN OF ACTIVE DUTY SERVICE MEMBERS: Current Military Dependent ID – must provide copy of front and back (must show service member as sponsor) or State-Issued Birth Certificate (must list service member as a parent) and a copy of active duty parent’s orders
- CHILDREN OF GOLD STAR FAMILIES: Current Military Dependent ID – must provide copy of front and back (must show service member as sponsor) or State-Issued Birth Certificate (must list service member as a parent) and a copy of late parent’s DD-1300.
